For a while on the HTSD tour they also had a cover slot, but I don't think they do that anymore. On the European arena leg they had one more slot for HTSD songs which rotated between Confusion and Dream No More (and somewhat rarely ManUNkind), bringing the total of "new" songs to 7. If they played a second night at a venue, they'd rotate songs and specifically not play Spit Out the Bone.

It's honestly kind of fascinating how Lars creates Metallica's setlists. There's definitely some thought put into it. Indeed, one might say he puts more thought into setlists than he does drumming these days!
 
He used it on Sad But True on the Black Album tour. I never really got why because I don't think there's any reason he'd need to use it, but he did. Judging by the string sizes it doesn't look like it's tuned like a standard six string bass either.
