This song's structure is like a small movie.
The way I understand it, Locomotive Breath is a close up plan to 2 people, "his woman and his best friend", having fun in bed.
Then progressively to long shot from high and above, the locomotive becomes his life getting out of control for him the "all time loser" seeing his children jumping at the stations and the train couldn't slow down..
A masterpiece!
