I get this reply sometimes too. It means, you are on the edge with your description. Example: They don't like Bruce's surname, so I can't get good Bruce's pictures. Sometimes you can get "I don’t want to see this" completely left field. Example: I wrote, Jack Nicholson peeks/peeking through smashed door. I understood that algorithm did not liked word "peeking". I changed peeking to looks/looking, and problem was solved. In other words, often You will intuitively see, which words trigger negative response. Anything that can be interpreted as adult content, etc.
Regarding last Scorpions cover, i do not understand why website's algorithm did not hid image immediately after generation. In other cases it was just blank image with words "image contains adult content" even if I only wanted to see Garden of Eden.
